Hearing “herniated disc” can feel alarming, especially with the assumption that it automatically means surgery. In reality, physiotherapy is often the first and most effective line of treatment, and many patients recover well without ever needing a surgical consultation.

What a Herniated Disc Actually Is

The discs between your vertebrae can bulge or herniate when their outer layer weakens or tears, sometimes irritating nearby nerves. This can cause localized back pain, or pain that radiates into the leg (similar to sciatica) if a nerve is involved.

Why Physiotherapy Is Often the First Step

For most herniated discs, conservative treatment — physiotherapy, activity modification, and time — leads to significant improvement, since the body has a real capacity to reduce inflammation and even partially reabsorb disc material over time. Surgery is generally reserved for cases with severe or progressive symptoms that don’t respond to conservative care.

How Treatment Works

Your physiotherapist will assess which movements and positions ease or aggravate your symptoms, and build a program around that — often starting with positions and gentle exercises that reduce nerve irritation, then progressively building core and spinal stability to support long-term recovery and prevent recurrence.

When Further Referral Is Needed

If you experience significant weakness, loss of bladder or bowel control, or your symptoms are rapidly worsening despite treatment, your physiotherapist will refer you for further medical assessment rather than continuing conservative care alone.
